We’ve made it easy for you to complete your Patient Information and Health History forms securely online, without having to create an account. Completing these forms before your appointment will save you time in the office.

Step 1: Access the Forms

• Click here to open your secure Online Patient Forms. Please have your medical and dental history and insurance information ready for easy entry.

Step 2: You’ll find the New Patient Information form and the Health History form, as well as the Smile Evaluation.

• Fill in all required fields in the New Patient Information form.
• On the Health History form, provide details about: Current medications Allergies Past surgeries Any medical conditions that may affect your dental treatment.

• Smile Evaluation: Let us know what you love about your smile and what you may want to improve. This gives insight into your personal goals and helps shape your treatment plan.

Step 3: review and submit

• After completing each form, click Next on the bottom right to proceed to the next form.
• Once completed, review all the information to ensure accuracy.
• Hit the Submit button, and your forms will be securely sent to our office.
